COVID-19 Vaccination Clinics

The Mason City School District is hosting with the Warren County Health District a COVID-19 vaccination clinic.  This vaccination clinic is being open to students who are 12-years old and older and their families from the Mason City Schools and Kings Local Schools. The clinic will offer the Pfizer vaccine for both doses. There is no cost to the student, family, or school district for the shots. 

Permission from a parent is mandatory for anyone under 18 years old. If the parent is with their child, they can grant permission during the clinic. If the student will be coming to the clinic without a parent, this consent form must be filled out and signed by the parent and brought with the student on vaccination day.

1st Dose

   ·  Saturday, May 15 from 8-3:30 PM at the Mason Middle School Arena, 6370 Mason-Montgomery Road, Mason OH 45040

   ·  Friday, May 28 from 8-3:30 PM at the Mason Middle School Arena, 6370 Mason-Montgomery Road, Mason OH 45040

2nd Dose

   ·  Saturday, June 5 from 8-3:30 PM at the Mason Middle School Arena, 6370 Mason-Montgomery Road, Mason OH 45040

   ·  Friday, June 18 from 8-3:30 PM at the Mason Middle School Arena, 6370 Mason-Montgomery Road, Mason OH 45040

The May 15 clinic can be scheduled at this time. You will be able to sign up for the May 28 clinic beginning on May 16th. Second dose appointments will be made with the nurse during the first dose vaccination clinic.

Sign up for the May 15th 1st DOSE Clinic appointment at https://gettheshot.coronavirus.ohio.gov/. Please use the access code: BP6KNCIAP9 and enter Mason, Ohio in the Location search.      

We are thankful for our partnership with the Warren County Health District. Feel free to share this information with anyone you know who needs the vaccine. 

The clinic will be at the Mason Middle School.  The students and families will need to park in the back of the building, closest to the football field and enter doorway C1.  It is best to enter off of Tylersville Road.
